**By signing above you have agreed to the following waiver information. I hereby acknowledge that my child is medically fit to participate in Precision Basketball Camp. I waive and release Precision Basketball from all liability for any injury or illness that may occur at camp.

Session 1 (8:00-9:30) - Boys entering grades 3-4

Session 2 (9:30-11:00) - Boys entering grades 5-6

Session 3 (12:00-1:30) - Boys entering grades 7-9

Session 4 (1:30-3:00) - Girls entering grades 3-7
